Pfizer discovers, develops, manufactures, markets, and distributes biopharmaceutical products. It sells mainly through wholesalers, while retailers, hospitals, clinics, government agencies, and pharmacies also buy its products. Insurers, government buyers, and other healthcare payers influence which treatments patients can access through formularies, contracts, and rebates. Pfizer earns revenue from product sales, royalties, alliances, and shared profits. Its established medicines and vaccines generate cash, while new products, research, and acquisitions must replace sales lost when patent protection ends and cheaper competitors can enter.

In the second quarter of fiscal year 2026, revenue reached $15 billion, up 3% as reported and about 1% operationally. Launched and acquired products grew 18% operationally. That makes Pfizer worth understanding now because replacement activity is visible, but the newer portfolio has not yet shown that it can replace the full revenue, profit, and cash contribution of older medicines.

Eliquis is an established medicine facing permitted generic entry in the United States from April 2028. Patent protection ending can bring lower prices and volumes, reducing revenue, margins, and cash. Pfizer therefore needs approvals, adoption, and acquired-product retention before exclusivity losses weaken its established cash engine.
